Youth and Engagement (Outreach)

The Engagement and Transition Program is an outreach-based, individualized case management service providing specialized wraparound support for youth.

This program supports youth who require additional assistance prior to placement at AYS, are transitioning out of AYS, or need stabilization within their current living situation. Each youth works closely with a YET Coordinator who collaborates with the youth and care team to develop an Individualized Service Plan (ISP). The ISP identifies strengths, outlines needs, and sets personalized goals.

Using a person-centered, trauma-informed, and attachment-based approach, the YET Coordinator supports youth in maintaining natural support systems and meeting their cultural, spiritual, and identity-related needs. Coordinators also assist with:

  • Scheduling and attending health-related appointments

  • Connecting youth with external & required resources and programs

  • Securing funding and financial setup for transitioning youth

  • Connecting youth to appropriate and sustainable housing options

  • Ensuring basic needs are met

  • Developing individualized strategies to achieve identified goals

Our focus is on reducing barriers, strengthening supports, and guiding youth toward stability, independence, and long-term success.
